A new global analysis puts a number on something most high-income countries assume they solved in the 2000s. Published in The Lancet Public Health on September 8, 2026, the first dedicated Global Burden of Disease assessment of secondhand smoke covers 204 countries across 1990 to 2023.
The headline finding is that progress is real and the total is not shrinking. As a company that sells air filters, we will be direct about what filtration can and cannot do with this particular pollutant, because the honest answer is less flattering to us than the marketing version.
In 2023, an estimated 2.71 billion people were exposed to secondhand smoke involuntarily, a global prevalence of 33.9%. That includes 767 million children under 15 and 1.49 billion women. The analysis attributes 1.66 million deaths and 44.8 million years of healthy life lost to that exposure in a single year.
The leading causes of death linked to exposure are ischemic heart disease, chronic obstructive pulmonary disease, and stroke. Among children, lower respiratory infections such as pneumonia carry the largest burden.
Because rates and headcounts are two different things. Global exposure prevalence has dropped by roughly 22% since 1990, but population growth has absorbed the gain, leaving the absolute number of exposed people roughly the same.
The regional picture splits sharply. Latin America and the Caribbean cut exposure by about 38% since 1990 on the back of comprehensive smoke-free legislation, and Denmark and Norway posted declines near 49% and 44%. Central Europe, Eastern Europe, and Central Asia still sit at 37.7% prevalence, roughly 146 million people. Sub-Saharan Africa saw exposure nearly double. Globally, around 70% of people live without fully effective smoke-free legal protection.
Because legislation reaches workplaces, restaurants, and transport, and it does not reach living rooms. Smoke-free laws have changed public indoor spaces across much of the world, but they leave private homes to household rules.
The demographics in the data point the same direction: the burden falls hardest on women and children, who are the people most likely to be present when someone smokes at home. In several Southeast Asian and Pacific countries where male smoking rates are high, women's exposure prevalence runs close to double that of men, exceeding 60% in some areas.
No, and no responsible filter company should tell you otherwise. The 2006 Surgeon General's report is unambiguous: "Separating smokers from nonsmokers, cleaning the air, and ventilating buildings cannot eliminate exposures of nonsmokers to secondhand smoke." ASHRAE, the engineering body that writes the ventilation standards, reached the same conclusion, stating that no engineering approach, including advanced dilution ventilation or air cleaning, has been demonstrated or should be relied upon to control health risks from environmental tobacco smoke.
There are two mechanical reasons. Tobacco smoke is part particle and part gas, and a MERV-rated media filter is a particle device, so the gas-phase components pass straight through. And a central HVAC system moves air between rooms by design, which means it can carry smoke from the room where someone is smoking into the rooms where nobody is. CDC's guidance on this is that a 100% smoke-free indoor environment is the only approach that fully protects people who do not smoke.
Where Filtration Does and Does Not Belong in This Conversation
Set the expectation correctly and filtration is still worth doing, just not for the reason people want it to be.
Two and a half billion people is not a filtration problem, and pretending otherwise would waste everyone's time. It is a policy and household-rules problem, and the countries that moved the number moved it with smoke-free laws, not with equipment.
What filtration earns is the space around that decision: once the indoor air is not being actively polluted, a correctly rated filter running in a correctly maintained system does measurable work on everything else you breathe at home. Get the rule right first. Then get the filter right.
